Living with dementia in the community

The ‘Living with dementia in the community’ project developed risk assessment tools for older people with cognitive and related functional decline to assist them or their healthcare professional to identify aspects of everyday functioning that are difficult or troublesome.

The tools assess daily activities in relation to functional and cognitive decline, in order to ascertain the choices and decisions that can be made to maintain the best quality of life and independence.

Two booklets have been produced using six key community care and disease management principles:

  • Enablement/Independence
